What to Expect at Your First Visit
Your initial appointment will consist of a complete dental examination and a diagnosis and treatment options. A series of x-rays will be taken as a baseline evaluation for your first visit. If you
have had x-rays taken at a previous office, please have copies sent to our office before your visit, either by email or mail, or bring them with you. Additional x-rays may need to be taken, based upon your individual consultation in order to make a proper diagnosis.
New Patient Information Forms
New patients can save time during their first appointment by completing the Patient Registration form prior to their visit. Simply print out the Patient Registration forms, fill in the information requested, and bring the completed form with you to your appointment.
Please arrive to your first appointment 15 minutes earlier then your scheduled time to check in and process your information.
